Terms & Conditions

Please read the following terms and conditions carefully. By scheduling an appointment or engaging services with Peggy Cohen, LCSW, you agree to the terms outlined below.

Overview

Peggy Cohen, LCSW (“Cohen Counseling,” “I,” “me,” “my”) provides outpatient mental health services to adults and college students, including individual therapy and counseling for a range of concerns such as anxiety, depression, grief, trauma, and major life change. These Terms and Conditions govern your use of my services and this website.

The information on this website does not constitute professional mental health advice and does not create a therapist-client relationship. A formal therapeutic relationship is established only when intake paperwork has been completed, informed consent has been signed, and services have been initiated.

Therapeutic relationship

A therapeutic relationship with me begins only after:

  • You have completed all required intake paperwork
  • You have signed an informed consent agreement
  • Services have been formally initiated

Viewing or interacting with this website does not establish a therapeutic relationship, and no information submitted through this website should be considered confidential in the same way as communications within an established therapeutic relationship.

Appointments

I ask that you provide at least 24 hours' notice to cancel or reschedule an appointment. Sessions cancelled with less than 24 hours' notice may be subject to a late cancellation fee. Repeated no-shows or late cancellations may affect your ability to continue services.

I see clients in person Monday through Thursday, with a limited number of telehealth appointments available. Sessions are 45 minutes. I reserve the right to modify session times with reasonable advance notice and will make every effort to accommodate scheduling changes on your end whenever possible.

Fees, insurance, and payment

Payment is due at the time of service unless other arrangements have been made in advance.

I am in-network with a number of insurance plans, including Providence Health Plan, Anthem EAP, Magellan, Blue Cross/Blue Shield, Anthem Blue Cross, Carelon Behavioral Health, Cigna, Aetna, and United Health Care/Optum. You can verify your coverage before we begin through the Headway portal. You are responsible for understanding and verifying your own plan's benefits, copays, and deductibles.

Self-pay sessions are $125, payable by cash or Venmo.

If you have questions about fees or insurance, please reach out before scheduling your first session. I'm happy to talk through the options.

Confidentiality

All information shared in therapy is confidential. I will not disclose information about you without your written consent, with the following exceptions required or permitted by law:

  • When there is a risk of imminent harm to yourself or others
  • When there is reasonable suspicion of child, elder, or dependent adult abuse
  • When disclosure is ordered by a court of law
  • When required by applicable state or federal law

No emergency services

I do not provide emergency or crisis services. If you are experiencing a mental health emergency, please:

  • Call 911 or go to your nearest emergency room
  • Contact the 988 Suicide and Crisis Lifeline by calling or texting 988
  • Contact the Crisis Text Line by texting HOME to 741741

I am not available 24/7 and cannot guarantee real-time responses between sessions. If you have concerns about crisis situations, we will establish a safety plan together.

No guarantees of outcome

Therapy involves personal growth, exploration, and change. While I bring significant training and experience to this work, outcomes cannot be guaranteed. Progress in therapy depends on many factors, including your engagement, the nature of the concerns being addressed, and consistency of attendance.

I am committed to working collaboratively with you and will regularly check in about how the work is going and whether adjustments are needed.

Termination of services

Either of us may end the therapeutic relationship at any time. If you wish to end services, I encourage you to discuss it with me so that an appropriate transition plan can be made.

I reserve the right to end services if:

  • Continuing therapy is no longer clinically appropriate
  • Practice policies have been repeatedly violated
  • Safety concerns arise that cannot be adequately managed in an outpatient setting
  • Fees go unpaid

In such circumstances, I will provide referrals to appropriate alternative resources whenever possible.
